To register by mail, please download this registration form, fill it out, and mail it along with a check for $50 (registration fee) or the full cost of the camp week (listed on the form) to:

(Parent's health insurance must pay for illness that is treated while the child is at camp. Our camp carries limited accident insurance which pays for the cost of treating an accident if the parent's insurance does not).

Include name, dosage, & instructions. Medications must be labeled and in original container.

If Health History shows any problems affecting camp participation, you are required to have a physician exam at least 90 days prior to camp.

Payment information

A minimum $50 deposit must accompany this form.

Financial aid is available upon request for those who need assistance to attend Miracle Bible youth camps. Please contact us for more information about financial aid.

Miracle Bible Camp does not receive your payment information; it is sent directly and securely to our payment processor for your safety.

I Hereby:

  1. Affirm there is no need for a doctor's examination prior to camp based on current health or that such an exam will be obtained with recommendations supplied to camp staff.
  2. Authorize camp nurse to give medications based on the above instructions.
  3. Authorize qualified camp personnel to give emergency medical care and determine the need for a physician's service.
  4. Release Miracle Bible Camp, its staff and volunteer workers from any liability or claims which may arise related to my child's participation in programs or trips sponsored by Miracle Bible Camp.

Your submitted registration provides Miracle Bible Camp the authorization to use photos & videos of you and your registered group for promotional purposes.
